Coolsportz Booking Form:

Please complete the following booking form.
Please note we have a minimum number of 8 children per day. If this is not reached we will either offer a
full refund or your place can be transferred to our Warwick Tennis Club or Hneley in Arden Camp for no additional cost.

YOU MUST PRESS 'SEND' AT THE BOTTOM OF THE BOOKING FORM. PLEASE NOTE SOME MAC COMPUTERS DO NOT SEEM TO SHOW THE SUBMIT BUTTOM, IF THIS IS THE CASE PLEASE CONTACT NEIL DIRECTLY TO BOOK.

Once completed you will then be taken back to the Coolsportz Home Page (it will NOT confirm you have sent it). However, you will receive a confirmation email of your booking together with information on how to pay.

By Clicking 'agree' you agree to the following:	
1.            The Data Protection Statement above
2.     I understand that while the staff in charge of the groups will take all reasonable care of the young people, that they cannot necessarily be held responsible for any loss, damage or injury suffered to or by my son/daughter arising during or out of the activity. 
3.     I certify that my child is in good health and can participate in the daily activities involved and if they have any medication they may need to take that they will bring it with them otherwise they will not be allowed to stay with no refund offered.  
4.     I grant permission for my child to be given first aid or treatment at hospital.
5.     I agree to provide my child with a packed lunch (snack for morning session) and ensure they attend wearing appropriate sportswear.
6.     I agree to pick up my child on time and will notify the Coolsportz Manager if they are being picked up by someone else and  in writing if they are walking home.  
7.     I give permission for me/my child/ren to be involved in any publicity (including photographs/TV footage) with Coolsportz. I understand that the pictures or filming may be used for promotional purposes and advertised on websites and in the media. 
8.     I have read and understood the Coolsportz cancellation policy which applies to all bookings.
